Eat fresh organic food, exercise and rest is the best insurance.

    The financial corporations are scams.

I work in the industry. Since you already know you need surgery, first make sure that there are no pre-existing condition rules and that the surgery you need is covered.

Then you want to consider the deductible, co-insurance, out of pocket max, and premiums. How much is the surgery going to cost? You'll want to figure out which one is going to cover the most for the lowest premium.

    For example,

    Plan A
    $750 deductible
    90% co-insurance
    $1500 OOP max
    premium $300/Mo
If your surgery and follow-up care is going to cost $4K. You'll pay $1075 for services and $3600 for a year's premium. Total cost to you: $4675.
IF your surgery and follow-up care is going to cost $20K. You'll pay $1500 for services and $3600 for premium. Total cost to you: $5100.

    Plan B
    $2500 deductible
    70% co-insurance
    $10000 OOP Max
    $75/month premium
    $4K Surgery: You'll pay $3550 for services and $900 for premium for a year = $4450 total
    $20K Surgery: You'll pay $7750 for services and $900 for premium = $8650 total

This is a bit over-simplified, but its a baseline way for looking at choosing a plan.
