This survey is not very valid IMHO.

I say this because it contains Yes or No answers to questions where you might wish to submit Yes (but sometimes No because ...) or No (but sometimes Yes because...)

In other words it oversimplifies and is likely to make you out to be really messed up when you are not.

It would probably be better with scaled answers.
