I called my rants but lets call yours "Failure Reporting, Analysis and Corrective Action System"
I dont see anyone in this movement is doing this. You cant improve and advance if you dont know what you did wrong. Recently I read "most important thing" thread and for some reason I started thinking about "Failure Report". Maybe it is just me but on projects that I had I always did these analysis and I dont see it being done here. People just drop it and forget all about it and that is not a way to move forward.

I know that this is touchy topic but I really think this needs to be done. Things slip through and are forgotten and they shouldnt be.
