This time only 4 Republicans voted no. I have no idea why 22 of them would change their minds. There were also 67 Democrats who voted "yes" the first time and there were only 15 who voted "yes" this time. I don't understand how there could be that big of a difference in one vote.

Those are procedural votes. They haven't voted on the Patriot Act extension.

Right, but I would think that if you're opposed to the Patriot Act you wouldn't vote to proceed with it.

Not that simple. Procedure is supposed to ensure a fair hearing and debate prior to adoption. Too overly anti-procedure and it's like opposing transparency and a fair hearing and full public debate.

"You broke down procedure so we didn't have a chance to debate. Now it will be passed along straight party lines with no discussion."

Sometimes you can use procedure to throw in a logjam, but usually only in the event of a significant chance of actual victory should you do something like that. Obstructionist winners are hated but tolerated. Obstructionist losers are despised. Pick your battles carefully, and always fight to win, (even when it's a sole dissent) and it'll be easier to stay on the right path.

I can totally see following the caucus on procedure, unless they turn around and do something messed up procedurally.
