Quote from website:
"The forms found on this page can be used by Local Health Officers when they find it necessary to isolate or quarantine individuals. The forms are in Word format. To use the forms, open a copy, save it to your computer and complete it there."

This website has it all. By that I mean all the signs of a totalitarian government attempting restricting people's freedom of movement and of course the freedom to not be injected with poisonous vaccinations. Everything from Voluntary forms to methods of dealing with those who will not voluntarily comply. Heck scroll down further and you'll even see they created a way extend you're involuntary incarceration. Ah, how times have changed.

Here's one of the "voluntary forms": http://www.doh.wa.gov/cfh/TB/Manual/Forms/VoluntaryDetention.pdf

Read it closer though and you will notice it says:

"The Health Officer may seek your voluntary compliance, may mandate isolation or quarantine, or may petition the Superior Court for an order authorizing isolation or quarantine or continued isolation or quarantine for a period up to 30 days"

Here's for New York City: http://www.newyorkcity.feb.gov/NPS%20Pandemic%20Influenza%20Preparation%20&%20Response%20Plan.pdf

It seems New York City really is playing no games especially after reading this:

"The President also could use the Insurrection Act (see above) and use the Armed Forces or
federalized National Guard to help suppress violence arising out of a State quarantine."

For those unaware the Posse Comitatus Act was the former law which disallowed/strictly limited the use of Military forces for law enforcement. This was completely repealed in 2008 essentially reverting back to the Insurrection Act which was originally passed in 1808.
Some of these documents are over 50pages long so I advice you use the keyword search by pressing control+f I'd suggest the keyword.. Quarantine it comes up quite a bit. -.-
