Well, the hospital can't make a profit, so they don't really have an incentive to do well....

cause they can't make a profit, what do they care?
In a free market, the hospital would make the reforms and then reap the rewards through
1. advertising
2. doing well on 3rd party hospital accreditation, reviews, and consumer reports.
