I would assume that Palin goes to the top of the ticket and she gets to pick her VP, but I'm not sure exactly what the official process is.
Is there hope for a Palin/Paul ticket if McCain kicks the bucket?

The world is a better place.

They would chose someone else to be the nominee and it wouldn't be Palin. The odds of it being Paul would be very low. They would give it to anyone but him.

I think it depends on how much time before the election. After a certain point, changes can't be made, and dead people have won elections before. At that point, the normal procedure for replacing someone who died in office would go into effect (which in McCain's case means the vice-president would take over.)
